- The current address where you are working
- Who the manager is; How many people are employed in the organisation; Your hours of work; how many hours of work are you employed; Are you employed, self-employed, full time or part-time.
- Whether you are receiving any benefits (if so please provide evidence)
- Whether you have any registered disabilities or learning needs (as we can help)
If you are not a UK Resident, we will also need your proof of indefinite leave to remain; your ID Card; and the Home Office's right to work and the right to be educated certificate/letter
